I endeavour to treat all personal and confidential information with equal care and respect.

- I collect the personal information from you during consultations for the purpose of providing the best treatment for your conditions. This would consist of Name, Phone number, Address, E mail, Doctors information and medical history. All information that is supplied to me by you is only kept on paper which is then filed in a locked cabinet.
- All children under the age of sixteen must have the consent of a parent or guardian they must also be accompanied by said person throughout the treatment.
- No medical records are kept on a Data base.
- The limited personal information kept electronically is so I may easily communicate with you using text, phone and email. I am the only person to have access.
- Transfer of personal data as part of clinical referral is only undertaken after additional consent. Any referrals are kept within the clinic you attend and they will act as a processor.
- All Information will be kept for periods as defined in our retention policies (See below)
- Reasonable measures are taken to protect personal information from access by unauthorised persons.
- Clients have the right of access to the records that I maintain, along with explanation of any notations.
- When sending an e-mail via my web site, I do ask persons to not divulge any medical or personal information, but to ask for a personal call. In the event you do add personal information I can assure you the provider is also compliant and committed to uphold the GDPR data privacy laws.

When I store information, I keep it for the following periods:

- After two years of non-attendance, records will be kept in a separate part of the cabinet.
- After three years non-attendance, records will be archived and kept in another locked cabinet at a separate location to which only I have access.
- Records are kept for 10 years for adults in line with our code of ethics. After that time they will be destroyed.
- For a child under the age of 16, records must be kept until they reach the age of 25. (This is for the protection of vulnerable groups regulations)
- Also after ten years all contact details are also deleted from the data base
Your Rights
At any time you the client may have access to you medical records on request. You have the right to ask for any of the content to be rectified that you may feel is incorrect.
If you no longer wish to attend the clinic and want to take a copy of your medical records, you may do so; at this time you will be asked to fill out a medical record request form.
